Position Title: Community Access Navigator Position

Background:
The Ma Mawi Wi Chi Itata Centre is seeking an individual for the Community Access Navigator position within the Wi Che Win, Beaver Medicine Bundle and the Indigenous Women’s Programs. This is an integral part of our Housing First homelessness model. The successful candidate will work intensively with participants to determine eligibility for services in the programs. This position reports directly to the Wi Che Win Program Manager.
Duties/Expectations:
- Administer a culturally appropriate intake and assessment process for prospective participants of the Doorways Collaborative;
- Market the Doorways Collaborative with all access point referring organizations;
- Develop and maintain constructive and effective contacts in the service systems that provide assistance to individuals experiencing being homeless;
- Provide accurate and useful information and support to persons who connect with Doorways for information on resources/services for those experiencing being homeless;
- Establish effective community relations relevant to the program;
- Develop clear work processes for moving people through intake;
- Provide information sessions to meet potential participants and assist with the application;
- Coordination of VI-SPDAT pre-assessment forms to assess direction of further contact with individuals;
- Receive VI-SPDAT forms - process and provide effective and timely evaluation and eligibility assessment of applications for Housing First;
- In person - assess basic eligibility and ensure appropriate placement into an Intensive Case Management or Assertive Case Management team;
- Arrange appointments to conduct SPDAT interviews;
- Contact with collateral agencies to explore needs of person based on information shared in the intake process;
Qualifications:
- Three-year degree in human services, social work or community planning, with an emphasis on assessment and meeting the needs of vulnerable populations that may include such disciplines and fields of study such as: social work, rehabilitation practitioner, community development;
- Minimum three years’ experience in case management, community outreach, mental health, addictions or housing programs in a front-line capacity is preferred;
- All required documentation needs to be up to date Certifications Credentials/Professional Registration;
- Exposure to or direct work history with principles of evidence-based practices, relationship/client centered approached, housing first principles and harm reduction models;
- Prior experience on meeting the needs of vulnerable populations relative to housing is preferred;
- Prior work with Indigenous cultures is preferred;
- Mental Health First Aid; Standard First Aid and CPR; Non-Violent Crisis Intervention and Suicide Intervention Certification preferred;
- Valid Manitoba driver’s license, clear driver’s abstract, and access to a vehicle for work related duties is required;
- Successful candidates will be required to provide a current and satisfactory Police Information Check including a Vulnerable Sector Search, and a current and satisfactory Child Welfare Intervention Record Check;
