Position Summary:

To provide day-to-day support for residents of Siloam Mission’s, 210 Evanson location, and to
coordinate and deliver an individualized package of support services to certain residents in
order to help them reach their potential. The Residential Support & Transition Services
Coordinator’s job will include coordinating and participating as needed in assessments, crisis
intervention, access to social services, the development of goals and a plan for reaching those
goals, acquiring access to basic needs, advocacy, and referrals. As part of Siloam Mission’s
mandate to be a connecting point between the compassionate and the less fortunate, you will
be expected to facilitate volunteers whenever possible in the performance of your duties.

Key areas of responsibility:

  • Provide orientation for new residents
  • Maintain files on residents, and appropriate statistics
  • Coordination and promotion of social programs and resident service hours on-site
  • Connect residents with appropriate community services by assessing their need and
    acting as a liaison between clients and agencies; arrange transportation for residents to
    and from related appointments
  • Act as the point person for the 210 Evanson team with all outside agencies and service
    providers, including law enforcement and emergency departments
  • Perform monthly room inspections and ensure compliance from the residents with the
    cleanliness standards of 210 Evanson.
  • Monitor the physical and mental health condition of residents, ensuring that basic needs
    such as medical attention and meals are being met.
  • Monitor video files pertaining to incidents
  • Assist Transition Services clients in designing and pursuing a positive life path of their
    choice by:
    • Maximizing their abilities for self-determination by enabling them and
      empowering them in decision-making
    • Providing assistance with goal setting, planning, and follow-through of identified
      goals
  • Assess clients’ relevant skill strengths and development needs
  • Act as an advocate for the needs of the clients of Siloam Mission
  • Monitor, evaluate, and communicate progress of clients with the clients themselves,
    team members, and the supervisor’s on a regular basis
  • Available to take charge when the Manager of 210 Evanson is absent
  • Other duties as assigned by Supervisor

Knowledge, Skills, and Abilities

  • Bachelor’s of Social Work or other social sciences degree or significant experience in the field.
  • Demonstrated ability to work as part of a team.
  • Ability to work within a harm-reduction framework
  • Experience in the mental health and/or addictions field
  • Experience in case management, completing assessments, and goal-setting
  • Excellent interpersonal skills and the ability to form relationships with marginalized
    individuals
  • Knowledge of community resources for homelessness, health, mental health and  substance use
  • Demonstrated understanding of the unique issues and barriers affecting the less fortunate
  • Highly developed understanding of and proven ability to communicate appropriate
    professional boundaries
  • Must have a valid driver’s license, a vehicle in good working order and be willing to
    drive clients to appointments

Additional Information:

  • Full-time, Monday-Friday, with occasional weekend and evening hours
  • Reports to the Manager of Transition Services
