Position Summary

The Rehabilitation Worker is the primary staff person responsible for providing service to participants. Rehabilitation
Workers use Psychiatric Rehabilitation, Housing First and Harm Reduction principles and approaches as well as other relevant tools and methods of responding (i.e: Stages of Change, COD tools, Carkhuff, Motivational Interviewing) to assist participants to gain skills and resources that assist them to choose, get and keep housing in the community. Other goal areas may include meaningful roles related to working, learning and socializing. The majority of the work takes place in the community working with individuals who have experienced chronic homelessness. The work often occurs in participant’s homes and homeless shelters.

A. Primary Accountabilities

  1. Assess and plan action with the participant around: readiness to make change, setting self determined goals, acquiring critical skills and resources and co-ordinating supports.

Performance Measurement Factors:

1.1 Effectiveness of partnerships measured by self-assessment, review of taped sessions, supervisor assessment and participant assessment.
1.2 Quality of the assessment and plan measured through supervisor review of file documentation and workpath utilization.
1.3 Time utilization measured through supervisor review of direct service statistics.
1.4 Timeliness of assessment and planning compared to program expectations for completion as measured through regular supervisor review of Rehabilitation Plans and other file documentation.

  1. Implement plans with and on behalf of participant related to: attaining self-determined goals, developing critical skills and resources, accessing, researching and co-ordinating resources, services and supports.

2.1 Effectiveness of partnerships measured by self-assessment, review of taped assignments, supervisor assessment and participant assessment.
2.2 Effectiveness of interventions compared to planned outcomes measured through supervisor review of taped sessions, file documentation, participant feedback and self-assessment.
2.3 Time utilization measured through supervisor review of direct service statistics.
2.4 Timeliness of skill and resource interventions compared to planned time frames and program expectations measured through regular supervisor review of file documentation, Rehabilitation Plans and management information system data.

  1. Assist significant people in the participant’s goal environment to work with the participant to achieve success and satisfaction by: identifying areas of difficulty, facilitating problem solving, providing information, coaching.

3.1 Thoroughness measured by self-assessment, participant assessment, supervisor review of file documentation.
3.2 Effectiveness of intervention measured through self-assessment, participant feedback, significant person feedback (if possible) and supervisor review of file documentation.

  1. Assess and plan action with the participant around: risk estimation, crisis prevention and recovery as well as eviction prevention.

4.1 Quality of the plan measured through supervisor review of file documentation.
4.2 Effectiveness of the plan measured through self-assessment, participant feedback and supervisor review of file documentation.

  1. Plan for closure and transition with the participant.

  1. Demonstrate a commitment to Housing First, Harm Reduction and Psychosocial Rehabilitation models, principles and philosophies.

B. Other Accountabilities

  1. Develop and maintain collaborative relationships with co-workers and external persons, programs, departments and agencies.
  2. Participate in supervision, staff and program and professional development activities. 
  3. Participate in program and agency meetings and committees.
  4. Maintain file documentation and program statistics in prescribed manner.
  5. Participate in peer mentoring and orientation with new staff, students and/or volunteers.
  6. Conduct program intake.
  7. Conduct housing search.
  8. Conduct employment search.
  9. Work with other staff to implement and monitor plans. This may include staff such as Housing Coordinator and Peer Outreach Worker.
  10. Demonstrate competence in understanding cultural diversity and work effectively with individuals with various cultural backgrounds.

Job Specifications

A. Technical Competencies

Education/Experience

This position requires three years completed post secondary education at the undergraduate level in human service related disciplines such as Social Work, Psychology, Sociology or Education and a minimum of two years direct service experience in a community based organization working directly with a clientele that would be defined as chronically homeless.
